Your Guardian PPO Dental insurance plan has a yearly maximum coverage amount which will vary by plan as well as an annual deductible. Services are covered on a percentage basis under three categories: Preventative, Basic and Major. Preventative services normally have the highest coverage ranging on average from 80% to 100%. Cavity detecting bitewing x-ray’s, routine cleanings, oral exams, sealants and fluoride treatments are generally considered preventative services. Basic services may include amalgam and composite fillings and basic extractions. Coverage for basic services may range from 60% to 80%. Major services like dentures, partial dentures, crowns, and bridges are generally covered at 30% to 50%.
Oral surgery, root canal therapy, and periodontal deep cleanings usually fall under the basic category, but depending on the individual insurance plan may be considered major services. Some Guardian PPO Dental individual insurance plans have waiting periods. This period may be as little as six months or as much as one year and usually involves basic and or major services.
