United Healthcare PPO Pre-Authorization
The staff at Sunnyvale Dental Care will do their best to use the breakdown of benefits that we receive from United Healthcare PPO insurance to give you an estimate of your dental copay. Some patients find receiving a pre-authorization from United Healthcare PPO very helpful.
After Sunnyvale Dentist Dr. Antonius completes your examination, you will be presented with a treatment plan. You will be able to ask questions and mention any treatment goals that you have.
In some instances, the Sunnyvale Dental Care insurance coordinator will find it necessary to submit the treatment plan to United Healthcare PPO insurance for pre-authorization. The pre-authorization will tell you your remaining yearly benefit amount, if your annual deductible has been paid, if the prescribed treatment is a covered benefit, and if so, the amount of your patient copay, and the dollar amount that United Healthcare will pay towards the claim.
Pre-authorization takes a few weeks and is not required for most procedures. Processing it may delay treatment. We will let you know if United Healthcare PPO insurance requires any items on your treatment plan to be pre-authorized.
Pre-treatment estimates have an expiration date. The date will be on the estimate. According to United Healthcare, “Pre-authorizations are not a guarantee of payment.” Final payment is determined after the items on the treatment plan have been completed and the claim has been sent to United Healthcare. Approved treatment must be finished while coverage on the United Healthcare PPO dental insurance plan is active.
United Healthcare PPO insurance suggests that pre-authorizations be done for treatment that is $300 or more or prior to any major treatment such as partial dentures and veneers. We will receive a copy of the pre-treatment estimate and one will be available to you from United Healthcare. Once received, we will contact you to schedule your appointment.
Sunnyvale Dentist Dr. Gupta is a United Healthcare PPO insurance provider, therefore he will follow the United Healthcare PPO fee schedule when submitting your dental claims which will mean lower out of pocket cost for you when compared to the price of treatment from a non-United Healthcare PPO provider.
