The plan maximum renews on the renewal date of your benefit period. Humana plan maximums vary based on the tier selected by you or your employer and typically range from $2500 on the higher end to $1000 on the lower end. Certain treatments, such as elective and cosmetic procedures are not usually covered and therefore do not deduct from your maximum. Preventive care is often covered at 100% while a procedure such as a crown may only be covered at 50%. You will pay the percent not paid by Humana.
