Although the staff at Sunnyvale Dental Care will do their best to provide you with the most accurate estimate possible for your dental treatment, some patients appreciate having their treatment plan pre-authorized by Guardian PPO Dental insurance before they schedule their next appointment. (It is important to note that pre-determination of benefits takes a few weeks to process and may delay treatment.)
Sunnyvale Dentist Dr. Gupta is a Guardian PPO Dental insurance preferred provider. What does this mean for you? It means lower out of pocket cost when compared to receiving treatment from a dentist who does not participate in the Guardian PPO Dental network and who has not agreed to accept the Guardian PPO Dental insurance reduced fees like Dr. Gupta has.
What can you expect during your first visit? Sunnyvale Dentist Dr. Gupta will do a thorough comprehensive exam utilizing diagnostic tools including dental radiographs (x-rays) to evaluate your dental health and needs. He will then compile his findings into a treatment plan that will be printed and presented to you. You will have the opportunity to ask any and all questions that you may have. After you and Dr. Gupta decide on a course of treatment, you may schedule your next appointment or request that the Sunnyvale Dental Care insurance coordinator submit a pre-authorization form to your Guardian PPO Dental insurance.
Under most Guardian PPO Dental insurance plans, pre-authorization of services is not required but is recommended for services that exceed $300 or are considered major services such as crowns, implants and dentures. Contained in the pre-determination estimate will be your plan data including your remaining annual maximum, any unmet deductible, whether or not the procedure is a covered service, and if so what your cost share is, and what Guardian PPO Dental insurance will pay on your behalf. A few things to note are: According to Guardian Dental, “pre-determination is not a guarantee of coverage, each pre-determination estimate has an expiration date, and all approved treatment must be completed while the Guardian PPO Dental insurance policy is active.”
A copy of the processed pre-determination estimate will be mailed to our office and one is available to you directly from Guardian insurance. Once received treatment can be scheduled.
