Aetna PPO Dental Pre-Authorization
Although the staff at Sunnyvale Dental Care will do their best to provide you with the most accurate estimate possible for your dental treatment, some patients choose to have their treatment plan pre-authorized by Aetna PPO Dental before they schedule treatment. Pre-determination of benefits takes a few weeks to process and may delay treatment.
Sunnyvale Dentist Dr. Gupta is an Aetna PPO Dental preferred provider. What does that mean for you? It means lower out of pocket cost when compared to receiving treatment from a dentist who does not participate in the Aetna PPO Dental network and who has not agreed to accept the Aetna PPO Dental reduced fees like Dr. Gupta has.
What can you expect during your first visit? Sunnyvale Dentist Dr. Gupta will do a thorough exam utilizing diagnostic tools including dental radiographs (x-rays) to evaluate your dental health. He will then compile his findings into a treatment plan that he will present to you. You will have the opportunity to ask any questions that you may have. After you and Dr. Gupta decide on a course of treatment, you may schedule your next appointment or request that the Sunnyvale Dental Care insurance coordinator submit a pre-authorization form to your Aetna PPO Dental insurance.
Under most Aetna PPO Dental insurance plans, pre-authorization of services is not required but is sometimes recommended for services that exceed $300 or are considered major services such as crowns and dentures. Contained in the pre-determination estimate will be your remaining annual maximum, any unmet deductible, whether or not the procedure is a covered service, and if so what your cost share is, and what Aetna PPO Dental insurance will pay on your behalf. A few things to keep in mind are: pre-determination is not a guarantee of coverage, each pre-determination estimate has an expiration date, and all approved treatment must be completed while the Aetna PPO Dental insurance policy is active.
A copy of the processed pre-determination estimate will be sent to our office and one is available to you directly from Aetna. Once received treatment can be scheduled.
